Eligibility Requirements

To qualify for assistance from this fund, applicants must meet all of the following criteria.

1
The patient was diagnosed with Lung Cancer within the last 5 years.
2
The patient is/was not a smoker.
3
Patient lives and owns their home within one of the following states: Arkansas, California, Georgia, Louisiana, Minnesota, Missouri, Pennsylvania, Tennessee, Texas, Utah, Virginia
4
Completed Cinnamon Helps Patient Authorization form. (Please submit authorization form when you submit your request.)

**This is a Health Equity fund as such there are additional geographic restrictions that apply.**
